TCM Care Manager Extender

This is a full time, direct care position. The primary responsibilities of the Extender are to:Perform general outreach, engagement, and follow-up with members.Coordinate services/appointments.Engage clients in health promotion activities.Share information with Care Manager on the member's circumstances.Make and track referrals.Participate in case conferences.Support the care manager in assessing and addressing unmet health-related resource needs.RequirementsMust be at least 18 years of age, have a high school diploma (or equivalent)1-2 years paid experience preferred within behavioral/mental health field.Spanish speaking/bilingual preferred.
