Speech-Language Pathologist

Overview Salary: $65.00 / hr Position PRN Speech‑Language Pathologist (SLP) Location Elderwood at North Creek – North Creek, NY 112 Ski Bowl Road North Creek, NY Position Overview The PRN SLP will provide high‑quality speech, language, cognitive, and swallowing therapy services to residents in a skilled nursing setting. This role requires adaptability, strong clinical judgment, and collaboration with an interdisciplinary team to deliver exceptional patient care. Responsibilities Evaluate, treat, and document speech, language, cognitive, and swallowing disorders in accordance with physician orders and plan of care. Develop and implement individualized treatment plans. Maintain accurate, timely documentation in compliance with regulatory standards. Collaborate with nursing, therapy team members, physicians, and families. Participate in care planning and interdisciplinary team meetings. Screen residents for communication problems and conduct in‑depth evaluations to make written recommendations for a therapy plan of care. Coordinate audiometric screening tests and refer for further audiology evaluations or treatments if necessary. Initiate speech‑language therapy plan of care according to physician orders and maintain required contact with the physician during the therapy program. Observe residents’ rights and treat residents with respect and kindness. Maintain written evaluations, progress notes, and discharge summaries for all residents in a speech‑language treatment program. Confere with other staff regarding special needs of residents and integrate the speech‑language treatment program with goals and approaches of other disciplines. Attend resident care planning and family/resident care plan review meetings for residents currently in a speech‑language treatment program. Maintain records including statistical reports about residents, in‑service education information, budget information, and material/equipment inventory. Submit statistical reports to the Business Office staff for Medicare billing payments and complete assessment forms upon request. Attend staff meetings and required or preferred in‑service training. Participate in developing and conducting in‑service training programs for facility personnel involved in resident care. Review written speech‑language policies and procedures annually and make necessary revisions. Maintain costs of the program within the parameters of the annual budget. Qualifications Current New York State license as a Speech‑Language Pathologist. M.A. or M.S. Degree in Speech‑Language Pathology from an approved school required. Certificate of Clinical Competence (CCC‑SLP) preferred; Clinical Competence Certification from ASHA preferred. Experience in a skilled nursing facility preferred. Strong clinical, organizational, and communication skills. Flexibility to work as needed based on facility demands. Ability to work independently and as part of a team. Current, valid CPR/BLS credential. Ability to communicate well verbally and in writing required. Ability to read, write, speak, and understand English at an intermediate or more advanced level. Verification of qualifications by the State Department of Health. Rehabilitation Therapies license number/certification number required.
