Contract (15 Weeks) | Full-Time | Days | Monday – Friday | Onsite

Key Responsibilities

• Provide physical therapy or physical therapy assistant services to patients in a rehabilitation setting
• Evaluate, develop, and implement individualized patient treatment plans in accordance with physician orders and facility protocols
• Document patient progress, treatment plans, and outcomes accurately and in a timely manner
• Collaborate with interdisciplinary care teams to support patient recovery and functional goals
• Maintain valid state licensure and carry a state or federal issued photo ID at all times while on site

Required Qualifications

• Active New Hampshire PT or PTA license in good standing
• Valid state or federal issued photo identification required to be presented at the facility
• Ability to wear a visible photo badge at all times while on site
• Ability to commit to a 15-week contract assignment

Preferred Experience / Skills

• Prior experience in a rehabilitation or skilled nursing facility setting
• Familiarity with documentation and EMR systems used in post-acute care environments
• Strong communication and team collaboration skills
