Registered Nurse (RN) - Care Manager

Case Manager Nursing

Schedule: 10-hour shifts, 4 days a week, no set day off, with every 6th weekend as needed.

Requirements:

• Bachelor of Science in Nursing (BSN)
• Minimum 3 years of experience in acute inpatient care and knowledge of current MS patterns of practice
• Extensive knowledge of methods for assessing physical/mental impairment and characteristics of illness
• Understanding of benefits, plan contracts, eligibility issues, and requirements
• Experience with Utilization Management (UM) and Care Management (CM) preferred
• Experience with EPIC EMR preferred
• Certification in Case Management (CCM) or related field preferred

Roles & Responsibilities:

• Assess, plan, implement, monitor, and evaluate options/services required to meet patients' health needs
• Use effective communication and available resources to promote quality, cost-effective outcomes for 18-22 patients
• Manage length of stay (LOS) and promote efficient utilization of resources
• Determine medical necessity and appropriateness of admissions utilizing InterQual screening and inpatient lists from Medicare
