Homeside Rehab is seeking a compassionate and dedicated Physical Therapist Assistant (PTA) or Occupational Therapy Assistant (OTA) to provide home-based therapy services in Pound Ridge, NY 10576 .
This is an excellent opportunity for a therapy professional looking for flexible scheduling, competitive compensation, and one-on-one patient care while working with a supportive clinical and administrative team.
Job ResponsibilitiesProvide skilled therapy services under the supervision and direction of a licensed Physical Therapist or Occupational Therapist
Carry out established plans of care and treatment interventions based on patient needs and goals
Assist patients with improving mobility, strength, functional independence, activities of daily living, and overall quality of life
Monitor patient progress and communicate changes or concerns to the supervising therapist
Maintain accurate and timely documentation of all patient care in accordance with regulatory and company requirements
Collaborate with supervising therapists and the clinical team to ensure coordinated and effective treatment
Communicate professionally with office staff, patients, and caregivers regarding scheduling and patient care
Active New York State PTA or OTA license/certification — required
Basic computer proficiency for electronic documentation
Strong interpersonal and communication skills
Ability to work independently in a home-based setting while following the established plan of care
Reliable transportation for home visits
Home care experience preferred
Minimum 1 year of experience in Home Health, Outpatient, or Skilled Nursing Facility (SNF) settings preferred
Job Type: Full-time, Part-time, Per Diem
Location: Pound Ridge, NY 10576
Pay: $65–$80 per hour
