Job Details Specialty: Home Health Nursing Job Type: Travel RN Assignment Location: Plymouth, Massachusetts (South Shore / coastal Massachusetts) Setting: On-site, with daily home visits and periodic office meetings Assignment Length: 13 weeks Start Date: August 31, 2026 Shift Duration: 8-hour days Schedule: Monday through Friday, 8:00 AM to 5:00 PM Weekly Estimated Pay: $1,880 to $1,980 Patient Volume: Approximately 5–8 patients per day Productivity Expectation: 30 points per week Travel Territory: Up to 75 miles from the assigned care center, including surrounding local communities Weekend/On-Call: Weekend rotation and on-call coverage may vary by assignment Documentation System: HomeCare HomeBase; OASIS proficiency required for RNs Job Requirements Current Registered Nurse (RN) license in hand Minimum 1 year of adult home health experience or 1 year of hospice experience , depending on specialty Recent experience with HomeCare HomeBase within the last 6 months preferred OASIS proficiency required for RNs Current CPR/BLS certification Reliable transportation, valid driver’s license, and auto insurance Willingness to travel daily to patient homes across the assigned service area Comfortable working independently and managing a full home health caseload Ability to complete documentation within 24 hours of each visit Open to on-call rotations and weekend coverage as needed Responsibilities Provide skilled nursing care in the home setting based on physician orders and care plans Complete start-of-care, wound care, follow-up visits, and other home health visits as assigned Assess patient needs, implement nursing interventions, and update care plans accordingly Educate patients and families on treatment plans, safety, and self-care techniques Coordinate care with the interdisciplinary team to support positive patient outcomes Supervise and evaluate home health aides and LPN/LVNs when applicable Travel between patient homes within the assigned territory and attend required meetings Document all care thoroughly and accurately in the EMR within required timelines Maintain compliance with productivity standards and assignment expectations Deliver compassionate, high-quality care that helps patients remain healthy at home Apply now to join a supportive Massachusetts home health nursing team and make a meaningful impact in Plymouth and the surrounding South Shore communities.
