Physical Therapist

Seeking a licensed Physical Therapist to provide personalized, one-on-one care to older adults in private homes and senior living communities. Key Responsibilities Include Conduct evaluations, develop individualized treatment plans, and deliver evidence-based therapy to improve mobility, independence, and overall well-being. Document patient progress accurately within electronic medical records. Collaborate with an interdisciplinary team including PTs, OTs, SLPs, and fitness specialists. Educate patients, caregivers, and staff on safety and aging-in-place strategies. Utilize telehealth and virtual care to enhance access and flexibility. Qualifications And Skills Valid Physical Therapy license or eligibility in the practice state. Degree from an accredited program. Proficiency in computer and EMR systems. Current CPR certification. Physical Requirements Ability to perform moderate physical effort, including lifting, repositioning patients, and prolonged standing/walking. Capability to drive to patient locations.
