Job Summary

Perform various direct patient care activities procedures and other indirect services under the direction of a Clinical Nurse.

Responsibilities

Deliver quality care to patients as delegated by the Clinical Nurse and according to hospital policy and procedure.

Assist with procedures as determined by hospital policy and procedure and regulatory guidelines.

Maintain a neat clean and safe patient and unit environment.

Provide optimum assistance to the medical and nursing staff by providing/ assisting patient with activities of daily living and daily assignments.

Assist in orientation of new Nursing Assistants.

Provide courteous and prompt response to patient requests.

May be required to cover the house.

Assist with unit maintenance related to ordering and receiving supplies medications manuals and equipment.

Provide constant observation required documentation and monitoring of assigned patients to ensure safety.
Receive report from exiting PSA or Clinical Nurse including specific patient behaviors to be monitored.
Ensures patient safety.
Promptly calls for assistance as needed.
Remains with patient at all times unless relieved by another staff member.
Notifies RN if IV tubes catheter or any items become disconnected.
Maintains a safe clean orderly and well-equipped environment.
Utilize Delirium kit if applicable to patient.
Reduce sensory stimuli (noise light television will only be on a patients request and to what patient requests to watch). Close door if necessary.
Communicate with family if family is visiting patient.
Provides total patient care within scope of practice (positioning tray set up/walk along side during ambulation bathing oral care changing linens turning every 2hours recording intake and output).
Ensures that all utensils that enter room are removed once patients meal is completed. patient is up in chair for all meals if applicable to patient.
Accompanies patient upon transfer to other nursing or non-nursing units when necessary.
Promotes infection control (hand washing alcohol gel use of personal protective equipment contact precautions and body substance isolation as indicated).
Notifies CRD if patient is transferring to another nursing unit.

The employee must possess/obtain (by the end of the orientation period) and demonstrate the knowledge and skills necessary to provide developmentally appropriate assessment treatment or care as defined by the departments identified patient ages. Specifically the employee must be able to demonstrate competency in: 1) ability to obtain and interpret information in terms of patient needs; 2) knowledge of growth and development; and 3) understanding of the range of treatment needed by the patients.

Competency Statement
