We are seeking a talented Transition Supervisor to join our team of nearly 500 employees. CCSI is contracted by Onondaga County Department of Children & Family Services (OC CFS) to provide program management services, including specialized staffing. Transition Supervisor is assigned to manage and oversee Transition to Independence (TIP) Team, comprised of transition facilitators (peer support or credible messengers). Position aims to ensure TIP team members provide high level of day-to-day delivery of services that meet or exceed quality standards of care to children and families, work collaboratively with providers and families to maximize treatment opportunities before, during, and after engagement.
In this role, you will
- Develop/support processes and procedures for TIP Team, ensuring: genuine relationship building and engagement with youth and families; deep dive needs assessments; comprehensive planning with providers, families, and youth; and effective referral and community services in place for child’s success in home, school and community settings.
- Provide supervision and coaching support to 4-5 Transition Facilitators, including addressing personnel issues and professional development of staff.
- Train and orient staff to their positions. Provide regular performance evaluations to ensure high quality service and develop individualized performance plans for team members. Provide individual supervision and case consultation.
- Lead or participate actively in (bi)weekly supervision, team case conferences, and staff meetings.
- Ensure development of plans of care that are youth-driven, culturally responsive, community-based and strength-based. Work with families, youth and providers to ensure all processes related to youth’s comprehensive plan occur in timely manner and include necessary treatment providers.
- Manage results of screening assessments to monitor linkages and service usage.
- Support general intake processes, such as interviewing and paperwork gathering, for youth referred to team.
- Collaborate with evaluator, project director, and county Onalytics to complete SPARS (SAMHSA’s Performance Accountability and Reporting System) data, SUPRT (SAMHSA Unified Performance Reporting Tools), RBA (Results Based Accountability) reports and any other written reports, statistics, and summaries in a timely manner and in accordance with program and agency standards.
- Facilitate training to staff of transition age youth serving organizations.
- Model System of Care (SOC) principles and practices in interactions with family, youth and other stakeholders.
- Perform other related duties as assigned.
What you bring
- MSW/Master’s Degree in related field plus 5 years of experience providing quality care coordination/case management services to youth and families, or equivalent education and work experience ( see CCSI's equivalency chart ).
- Proven experience supervising and managing diverse teams, including peer support or credible messengers preferred, but not required.
- Experience inputting and reviewing data from management information systems.

- Must possess a valid class D New York State Driver’s License and must have access to a motor vehicle in order to meet the transportation requirements of this position
- Satisfactory completion of required background screenings: criminal, Statewide Central Register, and driving.
