Youth HIV Care & Transition Case Manager

Washington University Youth HIV Clinic in the United States is seeking a Case Manager II to provide client-centered HIV case management for youth ages 13–29, including comprehensive bio-psychosocial assessments and care coordination with a multidisciplinary health care team. You will develop individualized care plans, coordinate referrals, document contacts, and support education on HIV/AIDS, confidentiality, and transition to adult care, while engaging with families and community partners. #J-18808-Ljbffr Washington University in St. Louis
