This travel, temporary assignment supports LTAC-relevant physical therapy services in a skilled nursing setting. Start date is September 28, 2026, and the assignment runs for 13 weeks. Responsibilities
- Provide physical therapy services to LTAC patients under supervision of a licensed physical therapist
- Conduct patient assessments and contribute to the development and modification of treatment plans
- Implement therapeutic exercises, gait and balance training, transfer training, and mobility work
- Monitor patient response to therapy, document progress, and communicate with the care team
- Ensure safe patient handling, adherence to infection control practices, and proper equipment use

Requirements
- Active Physical Therapy Assistant license in New Hampshire or eligible for licensure
- Experience in LTAC or skilled nursing rehab settings preferred
- Strong communication and collaboration skills; ability to document clearly
- Ability to work the assigned shift and travel schedule for a 13-week assignment
