Why Join Us Paid per visit – $38+ per visit Flexible scheduling – full-time, part-time, or per diem opportunities Mileage reimbursement 3 weeks PTO per year One-on-one patient care Referral bonuses up to $500 Annual merit increases with performance evaluations Full-time benefits: Medical, Dental, Vision, Life Insurance & 401(k) match after 1 year Your Role Provide direct patient care and case management in the home Conduct patient admissions and develop individualized care plans Obtain medical history and perform physical exams (vitals, mental status, diet, etc.) Deliver skilled nursing care in compliance with HIPAA Evaluate effectiveness of nursing services and coordinate total patient care Determine need for LPNs, Home Health Aides, or Personal Care Attendants Coordinate paramedical services (PT, medical social services, etc.) Supervise LPNs, HHAs, and Personal Care Attendants as needed Submit patient notes and documentation timely Qualifications Licensed RN Must speak Portuguese OASIS and WellSky (Kinnser) experience preferred (training provided) Med-surg and wound care experience preferred Physical exam within 6 months prior to first patient contact Current TB test (within 12 months) & CPR certification Valid driver’s license, auto insurance, and clean driving record Ability to attend 1–3 day orientation at Taunton office #J-18808-Ljbffr
