Pediatric RN Case Manager — Home-Based Palliative Care

Chapters Health seeks a Registered Nurse Case Manager (Pediatrics) to assess patient/family needs, coordinate the Plan of Care with the interdisciplinary team, and provide clinical and palliative care in home settings to keep patients in their homes as long as possible. Requirements include valid RN license in the state, 2+ years pediatric nursing experience (hospice/hospital preferred), EMR/EHR experience, and ability to travel to patient homes.
