Primary Care Advanced Practice Provider

Join a growing multi-specialty group dedicated to providing high-quality primary care in an office-based, 100% outpatient setting. This practice caters exclusively to adult patients (18+) and is supported by a full hospitalist service. Key Responsibilities Examine, obtain medical history, and treat acute and chronic illnesses. Collaborate with management, other physicians, and staff to ensure comprehensive patient care. Adhere to Patient-Centered Medical Home (PCMH) and Accountable Care Organization (ACO) initiatives and guidelines. Provide on-call coverage within the primary care group.
