Job Summary Responsible to facilitate care along a continuum through effective resource coordination to help patients achieve optimal health, access to care, and appropriate utilization of resources. This includes assessing patients for transition needs, identifying those at risk for readmission, conducting complex psychosocial assessment and intervention to promote timely throughput, safe discharge, and prevent avoidable readmissions. The role integrates national standards for case management scope of services, promotes transition management, readmission prevention, patient satisfaction, and maintains compliance with state and federal regulatory requirements, TJC accreditation standards, and Tenet policy. Education is provided to physicians, patients, families, and caregivers. Qualifications Minimum Education : Graduate of accredited school of Social Work, Master's Degree Social Worker (MSW) Preferred Education : Master's Degree Social Worker (MSW) Minimum Experience : None required Preferred Experience : 2 years acute care experience Required Certifications/License : Possession of current Texas State Social Work License based on Degree Preferred Certifications/License : Accredited Case Manager (ACM) #J-18808-Ljbffr Baptist Health System
