OVERVIEW

We are seeking a compassionate, mission-driven Nurse Practitioner to provide low-barrier, trauma-informed medical care to underserved and vulnerable populations throughout the Chicago area.

This role focuses on delivering care through a Harm Reduction framework in mobile outreach, street medicine, shelters, youth programs, and community-based settings while helping patients connect to ongoing healthcare and support services.

• MSN or DNP from an accredited program

• Illinois APRN License Required

• Family NP Preferred (Adult-Gerontology or related specialty considered)

• DEA License or eligibility to obtain

• Minimum 2+ years of clinical experience preferred

• Experience in:

  • Primary Care

  • Community Health

  • Harm Reduction

  • Behavioral Health

  • Homeless Healthcare

  • Substance Use Treatment

• Valid driver's license required

• Ability to work evenings/weekends if needed

• Provide medical care across:

  • Street outreach

  • Mobile health programs

  • Shelters

  • Youth/community programs

• Conduct focused medical assessments and treat acute/chronic conditions

• Initiate and manage medications including substance use treatment support

• Provide:

  • Wound care

  • Preventive care

  • Vaccinations

  • Health education

• Coordinate referrals to:

  • Primary Care

  • Mental Health

  • Specialty Care

  • Social Services

• Maintain EMR documentation and reporting

• Collaborate with interdisciplinary outreach teams

• Participate in quality improvement and program development initiatives

• Street Medicine / Mobile Health

• MAT (Medication-Assisted Treatment)

• Shelter or community outreach programs

• Working with underserved populations

• Spanish-speaking ability preferred

• Experience supervising students/volunteers is a plus

• Strong commitment to harm reduction and trauma-informed care

• Excellent communication and de-escalation skills

• Ability to work independently in nontraditional environments

• Strong documentation and organizational skills

• Cultural humility and patient-centered mindset

• Flexibility and adaptability in community-based settings

• Community-focused healthcare setting

• Mobile outreach and outdoor environments

• Collaborative interdisciplinary team

• Ability to travel between outreach locations

• Ability to stand/walk for extended periods

• Occasional lifting up to 20 lbs

• Comfortable working outdoors in varying conditions
