Nurse Practitioner - In-Home Health Assessments

Key Responsibilities Conduct thorough assessments for adults and geriatrics, including medical history review, diagnosis, treatment, health education, and referrals, all documented in line with Matrix protocols. Perform necessary point-of-care and lab-based testing. Collaborate with a network of healthcare providers including physicians, case managers, social workers, family members, and caregivers. Provide care directly in members' residences or skilled nursing facilities. Work closely with members' Primary Care Physicians to ensure continuous patient education and follow-up as needed. Benefits of Working at Matrix Enjoy the flexibility to schedule visits based on your availability. We conduct assessments from Sunday to Saturday, 8 am - 7 pm. Competitive pay structure with income based on the number of assessments completed. Those averaging 20-100 visits monthly can earn between $1,800 - $11,500, depending on location. Benefit options include mileage reimbursement, 401(k), employee referral bonuses, and limited voluntary benefits. Opportunities for obtaining additional state licensure if necessary. Option for transitioning to full-time roles with a sign-on bonus. Qualifications Master's Degree and completion of NP licensure. Active NP licensure in relevant state with prescriptive authority or ability to obtain such authority. AANP, AACN, or ANCC board certification in relevant specialties. Current BLS, ACLS, or CPR certification. Preferred one year of NP experience; new graduates are encouraged to apply. Valid driver’s license and automobile insurance meeting state requirements. Strong computer skills and proficiency with health/medical record software. Excellent verbal and written communication skills.
