Remote Care Navigator for Children & Families
CLCF
CLCF is seeking a Care Manager to provide compassionate support, advocacy, and care coordination for children and families facing medical, developmental, or emotional challenges. You will assess needs, develop individualized care plans, connect families with services, and monitor progress in collaboration with diverse partners.
This contract position allows remote work with monthly in-person visits across New York.
