In-Home Nurse Practitioner (Per Diem) - Topeka, KS

Job Summary Per Diem Nurse Practitioner – In‑Home Health Evaluations at Signify Health, part of CVS Health®. Responsibilities Conduct in‑home visits lasting about 45 minutes to evaluate patients, review medication and medical history, perform physical assessments, and order additional diagnostic tests as needed. Use iPad and clinical workflow tools; not required to prescribe medication or alter treatment regimens. Work with Signify Health as a clinician colleague to fulfill role requirements. Provide virtual visits when necessary. Offer diagnostic and preventive services such as spirometry, diabetic eye exams, DEE, etc. Travel locally and within licensed states; flexible schedules required. Qualifications Active, unrestricted license(s) in coverage area(s). Board certification (required). Multi‑state licenses (preferred). Pay & Benefits Anticipated weekly hours: 12. Part‑time. Pay range: $37.66 – $81.11 per hour. Per diem employees qualify for hourly base rate plus performance incentives; malpractice insurance coverage during Signify Health activities; supplies and other perks.
