Ozark Physical Medicine Center for Operations Home Health Case Manager RN (Healthcare) Job Summary The Home Health Registered Nurse (RN) Case Manager provides comprehensive nursing care to patients in their homes, ensuring quality, patient-centered care that promotes independence and improves health outcomes. The RN Case Manager develops, implements, and evaluates individualized care plans while coordinating services with physicians, therapists, social workers, home health aides, and other healthcare professionals. This position requires strong clinical assessment skills, excellent communication, and compliance with all federal, state, and agency regulations. Essential Duties and Responsibilities Perform comprehensive patient assessments upon admission and at regular intervals. Develop, implement, and revise individualized plans of care based on physician orders and patient needs. Coordinate all aspects of patient care with physicians, therapists, social workers, home health aides, and other healthcare providers. Provide skilled nursing services including: Medication management and administration Wound care and dressing changes IV therapy (if applicable) Catheter care Disease management and patient monitoring Assess patient response to treatment and modify care plans as appropriate. Educate patients and caregivers regarding disease processes, medications, treatments, safety, nutrition, and self-care techniques. Monitor patients for changes in condition and communicate findings promptly to the physician. Complete accurate, timely, and thorough clinical documentation in the electronic medical record. Ensure compliance with Medicare Conditions of Participation, state regulations, agency policies, and accreditation standards. Participate in interdisciplinary team meetings and case conferences. Coordinate patient discharge planning and community resource referrals. Supervise and delegate appropriate tasks to Licensed Practical Nurses (LPNs), Home Health Aides (HHAs), and other assigned personnel. Participate in quality improvement activities and performance improvement initiatives. Maintain patient confidentiality in accordance with HIPAA regulations. Participate in on-call rotation as required. Qualifications Graduate of an accredited school of nursing. Current Registered Nurse (RN) license in the state of Missouri. Minimum one (1) year of clinical nursing experience required. Previous home health, hospice, case management, or community health experience preferred. Current BLS (Basic Life Support) certification. Valid driver's license, reliable transportation, and current automobile insurance. Ability to travel throughout the service area. Knowledge, Skills, and Abilities Strong clinical assessment and critical thinking skills. Excellent communication and interpersonal skills. Knowledge of home health regulations, Medicare guidelines, and documentation requirements. Ability to work independently and manage multiple patient cases. Strong organizational and time management skills. Proficiency with electronic health records (EHR). Ability to educate patients and caregivers effectively. Compassionate, patient-focused approach to care. Physical Requirements Ability to stand, walk, bend, kneel, and reach frequently. Ability to lift and transfer up to 50 pounds with or without assistance. Frequent driving and travel between patient homes. Ability to perform repetitive hand movements for documentation and patient care. Visual and auditory acuity necessary to assess patient conditions. Working Conditions Work is performed primarily in patients' homes. Exposure to varying environmental conditions and infectious diseases. Frequent travel within the assigned service area.
