Case Manager (RN)
We are seeking experienced Registered Nurses (RNs) with strong assessment, care coordination, and member education skills to support a Disease Management program. Candidates with prior Case Management experience, telephonic care management experience, and Medicaid knowledge are highly preferred.
Required Qualifications:
- Active Pennsylvania RN license
- Disease Management experience
- Telephonic Case Management experience
- Strong assessment, care coordination, and member education skills
Preferred Qualifications:
- CCM (Certified Case Manager)
- Medicaid experience
- Prior Case Management experience
Additional Requirement:
- Candidates must complete the Care Manager Assessment in Glider as part of the hiring process.
