Home Health RN Case Manager

Full-time and PRN hours available! Registered Nurse Responsibilities Directly supervise Home Health Aides and LPNs, carrying out supervisory responsibilities in accordance with the organization's policies and applicable laws. Responsibilities include scheduling, planning, assigning, directing work, appraising performance, addressing complaints and resolving problems; conducting supervisory visits with Home Health clients and authorizing in‑services. Conduct client assessments, suggesting level of needed service packages to client and their family. Develop plans of care with the participation and consent of the client and/or caregiver/family member or legal representative. Set up medications for clients, monitor side effects, drug interactions, dosages, and implement changes per physician instructions, informing client and family of changes immediately. Order medication for clients from pharmacy. Listen and document significant changes in clients’ conditions, including incidents, unexplained injuries, medication errors, loss of client property, or evidence of client or family dissatisfaction, and communicate with supervisor. Analyze data gathered from assessment and records reviews, and act promptly to resolve problems. Participate in quality assessment and assurance activities, implementing recommendations and monitoring results. Develop plans of care with measurable objectives and timetables, accommodating clients’ needs and preferences, including choices about preferred intensity of medical treatment. Maintain ongoing communication with other disciplines/departments to evaluate client responses to interventions, suggest alternative approaches when indicated, and revise and update plans of care to reflect progress toward goals as directed by supervisor. Interact with clients, caregivers/family members, and legal representatives, resolving concerns. Identify discharge planning needs as part of the care plan development and implement prior to discharge of the patient. Communicate with the physician regarding the patient’s needs and report any changes in the patient’s condition; obtain physician orders as required. Instruct, supervise, and evaluate home health aide care provided every two (2) weeks or per regulatory guidelines. Demonstrate C.A.R.E. values to our clients, family members, customers and staff. Comply with and adhere to the appropriate use of Personal Protective Equipment (PPE) required by the Bloodborne Pathogens Standards. Protective Personal Equipment (PPE), including protection for eyes, face, extremities, clothing, and shields, will be provided, used, and maintained. Job Requirements Graduate of an accredited school of nursing with RN licensure in the state in which services will be provided. One (1) year of home health care experience preferred. One‑year supervisory/management experience with direct care staff preferred. Excellent observation, verbal and written communication skills, problem‑solving skills, basic math skills; nursing skills per competency checklist. Physical ability to walk or stand for prolonged periods, lift, position or transfer patients, lift supplies and equipment, reach, stoop, bend, kneel or crouch. Visual acuity and hearing sufficient to perform required nursing skills.
