Contract | Days | Expected 10 hours/week | Flexible schedule with at least 2 closing shifts per week until 7:00 PM

Key Responsibilities

• Complete comprehensive evaluations of assigned patients and develop, implement, and modify individualized treatment plans in accordance with patient needs, goals, and physician direction
• Complete medical record documentation in a clear, concise, accurate, and timely manner, including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry in compliance with state, federal, and intermediary guidelines
• Ensure all care is provided under signed, valid physician orders and within insurance approvals as required for patient care
• Design functional treatment programs that support effective patient return to community activities, with a focus on progressing patients toward discharge within an established length of stay
• Participate in student programs as assigned, providing appropriate supervision, developing learning objectives, and completing student evaluations
• Adhere to discipline-specific State Practice Act and all applicable regulatory requirements, maintaining professional licensure in good standing and completing all state-mandated continuing education requirements

Required Qualifications

• Active Physical Therapist license in the state of Ohio (or eligibility to obtain)
• Ability to evaluate patients and develop clinically appropriate treatment plans using sound clinical reasoning
• Commitment to patient and employee safety standards, including safe patient handling and infection control practices
• Adherence to professional codes of conduct and discipline-specific ethical standards

Preferred Experience / Skills

• Experience or coursework in orthopedic physical therapy
• Experience with outpatient clinic documentation systems and workflows
• New graduates are welcome to apply
