Home-Visiting Nurse: Family & Maternity Health Advocate

Texas Children's Hospital is seeking a Nurse Home Visitor for the Nurse Family Partnership. You will assess, plan, implement, monitor and evaluate services to meet health, psychosocial needs, and economic self-sufficiency for mothers and babies. You will provide in-person case management for high-risk members, coordinating care across the continuum to achieve optimal outcomes.
