Yearly refills

Diagnosis and treatment of erectile dysfunction

Kidney stones

Male incontinence

Cystoscopies

Surgical assist is referenced as applicable based on provider background

Required Qualifications

Nurse Practitioner (PA may be considered depending on surgical background)

Washington license in hand at time of presentation. IMLC is not accepted per Order notes

Certifications: BLS and ACLS

Recent clinical activity documentation or case logs within the prior 24 months are referenced
