The Social Worker must have obtained an MSW and meet the clinical license requirements for the state of AZ (ex. LCSW, LICSW)

The Social Worker will support DaVita Skilled Nursing Facility Dialysis (SNF) patients. Approximately 10% of all dialysis patients reside in a SNF and are transported to dialysis three times a week placing a significant burden on patients and adding significant avoidable costs to the health care system. SNF dialysis patients receive dialysis treatment onsite at the Skilled Nursing Facility in which they reside, instead of being transported to an outpatient dialysis center.

In this vital role, you will help patients understand their rights and responsibilities, and guide them in managing the physical, mental, emotional and financial demands of End Stage Renal Disease.

The MSW currently is likely to have a starting caseload of around 110, but this has the potential to grow. The MSW can work a hybrid schedule, but must be located in the Phoenix metro areaThe number of days on-site may vary. It is likely to be about two days/week from home and three days on-site meeting with patients at our Arizona Skilled Nursing Facility Dialysis locations. The MSW is likely to support patients at the following sites:

• DaVita SNF Dialysis At Haven Sky Harbor 1880 E Van Buren St Phoenix, AZ 85006
• DaVita SNF Dialysis at Palm Valley 13575 W Mcdowell Rd Goodyear, Az 85395
• DaVita SNF Dialysis at Haven Tucson 3705 N Swan Rd Tucson, Az 85718
• DaVita SNF Dialysis Plaza 1475 Granite Reef Rd Scottsdale, AZ 85257

Requirements:

• Master's Degree in Social Work (MSW) with a specialization in clinical practice from an accredited school of Social Work.
• Meets all state required regulations to practice in the dialysis setting.
• Obtained their Clinical License (e.g., LCSW, LICSW), meeting all licensure requirements for their state.
• Proficient in all Experienced level Social Worker duties.
• Intermediate computer skills.
• Demonstrated knowledge of government and private insurance programs.
• Excellent interpersonal and communication skills
