Director of Nursing (Home Health)

We are seeking an experienced and driven Director of Nursing (DON) to lead our clinical operations, support our field clinicians, and ensure exceptional patient care and regulatory compliance.

Position Summary

The Director of Nursing is responsible for overseeing all clinical services within the home health agency. This leadership role ensures compliance with federal, state, and accreditation standards while promoting clinical excellence, operational efficiency, and positive patient outcomes.

The ideal candidate is a strong leader with home health experience, excellent knowledge of Medicare regulations, and the ability to manage and mentor clinical staff in a fast-paced environment.

Responsibilities

• Oversee daily clinical operations of the home health agency
• Ensure compliance with Medicare, state, federal, and accreditation regulations
• Supervise and support RNs, LPNs, therapists, and clinical staff
• Review and monitor patient care plans, documentation, and outcomes
• Participate in QAPI/performance improvement initiatives
• Assist with survey readiness and corrective action plans
• Collaborate with intake, scheduling, billing, and administrative departments
• Monitor clinical productivity and staffing needs
• Provide clinical education, training, and mentorship to staff
• Ensure timely completion of OASIS documentation and physician orders
• Address patient concerns and resolve clinical issues promptly
• Support business growth while maintaining quality standards

Qualifications

• Active Registered Nurse (RN) license in good standing
• Minimum of 2–3 years of home health leadership or management experience preferred
• Strong understanding of Medicare Conditions of Participation and OASIS
• Experience with EMR systems and clinical documentation review
• Excellent leadership, communication, and organizational skills
• Ability to manage multiple priorities and lead interdisciplinary teams
• Current CPR certification required

Preferred Qualifications

• Previous experience as a Director of Nursing, Clinical Manager, or Administrator in home health
• ACHC, CHAP, or Joint Commission survey experience
• Knowledge of PDGM and home health reimbursement models

Schedule

• Full-time
• Monday to Friday
• No on-call rotation required

Work Location
