Wound Care Nurse RN/ LPN

Job Summary:

We are seeking a highly skilled and dedicated Wound Care Nurse RN/ LPN to join our team at Windsor Rehabilitation and Healthcare Center in Windsor, North Carolina. As a Wound Care Nurse, you will play a critical role in providing excellent patient care and wound management to our residents. Responsibilities:

• Perform skin treatments for all residents under your care, obtaining treatment orders from attending physicians and assisting with modifying treatment regimens as needed

• Complete head-to-toe body audits on new admissions and re-admissions to identify open areas and ensure implementation of skin sheets, treatment orders, and preventative measures

• Update skin sheets every 7 days and review residents with new skin alterations to ensure implementation of skin care treatment and care plans

• Provide wound care when needed and delegate prn

• Maintain accurate and timely documentation of wound care treatments and resident progress

• Collaborate with the wound physician to develop and implement individualized wound care plans

• Educate and supervise certified nursing assistants (CNAs) and other staff members on wound care principles and procedures

• Participate in departmental meetings and in-services as directed

Essential Functions:

• Complete daily, weekly, and ongoing responsibilities as outlined in the job description

• Maintain accurate and timely documentation of resident care and progress

• Collaborate with interdisciplinary team members to develop and implement individualized care plans

• Provide wound care and education to residents, families, and staff members

• Maintain a safe and clean environment for residents and staff

Requirements:

• Graduate from an accredited nursing program

• Current State License as a Registered Nurse or Licensed Practical Nurse

• C.P.R. Certified

• 0 years of experience in wound care or related field
