Description
  • Assess patients within both inpatient and outpatient environments and develop personalized treatment plans
  • Provide physical therapy sessions centering on mobility, strength, balance, and overall function
  • Work closely with nurses, physicians, case managers, and other rehabilitation experts to achieve optimal patient outcomes
  • Guide patients and families through therapeutic objectives, discharge processes, and at-home exercise regimens
  • Record evaluations, interventions, and progress in compliance with facility policies and regulatory standards
  • Engage in interdisciplinary rounds and conferences to ensure integrated care delivery
Education
  • Doctorate in Physical Therapy (DPT) or a related degree from an accredited institution
Certifications
  • Active state licensure as a Physical Therapist
  • CPR certification preferred
  • Specialty certifications (e.g., neurological, geriatric) are advantageous but not mandatory
Skills
  • Prior experience strongly preferred
  • Proficiency in managing patients with complex medical conditions and mobility challenges
