What You'll Do Complete skilled nursing visits in patients' homes Perform comprehensive nursing assessments Complete Start of Care, Recertification, Resumption of Care, and Discharge visits as assigned Provide wound care, medication management, and disease education Monitor changes in patient condition Educate patients and caregivers Coordinate care with physicians, therapists, and other members of the care team Complete accurate and timely documentation using WellSky What We're Looking For Active Virginia Registered Nurse (RN) License Graduate of an accredited nursing program Current CPR/BLS Certification Valid Driver's License and reliable transportation Strong assessment and communication skills Ability to work independently in a home health setting Home Health and OASIS Experience Preferred, but Not Required WellSky Experience is a Plus Why Human Touch?
