$89,000–$109,000 The Opportunity As a home health Occupational Therapist, you'll help patients reclaim independence in their own homes — evaluating functional abilities, designing individualized treatment plans, and delivering hands‑on rehabilitation focused on activities of daily living. Your work directly supports aging in place and meaningful recovery for patients with complex needs, including neurologic conditions. Your Day-to-Day Assess patients' functional abilities and develop individualized treatment plans centered on ADLs and IADLs Implement rehabilitation techniques with a focus on complex neurologic injuries and conditions Teach patients and caregivers adaptive strategies, use of assistive devices, and home modifications to promote safety and independence Provide skilled occupational therapy interventions in the home environment Collaborate with interdisciplinary team members including nurses, physical therapists, and physicians Document care, progress, and outcomes in compliance with regulatory and agency standards Travel to patient homes within the designated coverage area What We're Looking For Master's degree in Occupational Therapy (or equivalent) with active OT licensure or certification in Minnesota Minimum two years of independent clinical experience Specialized knowledge of rehabilitation techniques for complex neurologic conditions preferred Hands‑on experience in home health, medical‑surgical, or rehabilitation settings preferred Valid driver's license with the ability to travel across all service locations #J-18808-Ljbffr
