As a Social Worker at Life Stage Counseling, you will connect with clients through regular consultations and virtual visits, allowing you to address their needs effectively.

Key Responsibilities

• Conduct detailed psychosocial assessments for seniors to identify emotional, mental, and social factors affecting their health.
• Develop individualized care plans in collaboration with clients and their families, tailored to address specific goals, preferences, and challenges.
• Provide counseling and emotional support through critical life transitions, guiding seniors and their families with empathy and understanding.
• Advocate on behalf of clients to ensure they access necessary community resources, services, and support networks.
• Work closely within interdisciplinary teams comprising nurses, caregivers, and healthcare professionals to coordinate comprehensive care strategies.
• Conduct regular virtual visits to monitor client progress, assess living conditions, and address any concerns or safety issues proactively.
• Facilitate effective communication among family caregivers, resolving conflicts and enhancing care collaboration.
• Educate clients and their families about available resources, support groups, and programs that foster social engagement and improve quality of life.
• Maintain thorough documentation of client interactions, care plans, assessments, and progress notes while adhering to confidentiality standards.
• Stay updated on the latest social work practices and policies, ensuring compliance with ethical standards.

Qualifications

• A Master's degree in Social Work from an accredited institution.
• Current, valid licensure as a Master Social Worker (LMSW) in Florida.

• In-depth knowledge of social work principles, aging issues, and community resources available to support seniors.
