Registered Nurse (RN) Case Manager – Home Health

Location: Davidson & Forsyth Counties, NC
Schedule: Monday–Friday, 8:00 AM–5:00 PM
On-Call: None
Mileage Reimbursement: $0.43/mile
Productivity Requirement: 30 visits/points per week

Position Overview

We are seeking a compassionate and experienced Registered Nurse (RN) Case Manager to join our Home Health team serving Davidson and Forsyth Counties. In this role, you will provide comprehensive patient assessments, develop individualized care plans, coordinate care across multiple disciplines, and advocate for patients to ensure optimal clinical outcomes.

The ideal candidate has strong home health experience, excellent case management skills, and a passion for delivering high-quality patient-centered care in the home setting.

Key Responsibilities

• Perform comprehensive assessments of high-risk patients and identify healthcare needs.
• Develop, implement, and monitor individualized care plans in collaboration with physicians and interdisciplinary team members.
• Coordinate care transitions and ensure continuity of care across healthcare settings.
• Collaborate with primary care providers, specialists, social workers, discharge planners, and other healthcare professionals.
• Address barriers to care by coordinating non-medical support services, including transportation, housing resources, and community services.
• Monitor patient progress and continuously reassess care plans to ensure appropriate, cost-effective levels of care.
• Identify opportunities for health promotion, disease prevention, and improved patient outcomes.
• Prevent adverse patient events whenever possible and intervene promptly when issues arise.
• Maintain accurate and timely documentation of clinical, functional, and financial outcomes.
• Advocate for patients while ensuring privacy, confidentiality, and regulatory compliance.
• Complete OASIS assessments and documentation in accordance with agency and regulatory standards.

Qualifications

• Current, unrestricted Registered Nurse (RN) license in North Carolina.
• Associate’s or Bachelor’s Degree in Nursing or related field.
• Home Health experience required.
• Case Management experience required.
• OASIS experience required; OASIS certification preferred.
• HCHB (Homecare Homebase) experience required.
• Case Management Certification preferred.
• Strong clinical assessment, critical thinking, and decision-making skills.
• Excellent communication, organizational, and documentation abilities.
• Valid driver's license, reliable transportation, and ability to travel throughout the assigned coverage area.
