This role works closely with residents, families, and the interdisciplinary team to deliver individualized therapy interventions.Reporting: This position reports directly to the Director of Rehabilitation and collaborates with the rehabilitation team and interdisciplinary staff.Key ResponsibilitiesEvaluate residents’ functional abilities, mobility, and strength to develop individualized treatment plans.Implement therapeutic exercises and interventions to improve mobility, balance, and independence.Educate residents and families on exercise programs, safety techniques, and assistive devices.Document therapy services and resident progress accurately and in a timely manner.Collaborate with the interdisciplinary team to support comprehensive care planning.Promote a safe, comfortable, and supportive environment for all residents.Demonstrate compassion, respect, and professionalism in all interactions.Education and License RequirementsBachelor’s or Master’s degree in Physical Therapy from an accredited program.Current Washington State Physical Therapist license or eligibility for licensure.ExperienceExperience in skilled nursing or rehabilitation settings preferred, but new graduates and newly licensed therapists are welcome to apply.Qualifications and SkillsStrong communication and interpersonal skills.Ability to follow care plans and directions from supervising staff.Ability to work effectively in a team environment.Compassionate, patient, and committed to resident-centered care.Physical ability to safely assist residents with mobility and therapeutic exercises.How to Apply: Click Apply and complete the online application form.Please ensure you:Answer all application questionsUpload your professional license and any relevant documentsWe look forward to reviewing your application! #J-18808-Ljbffr Bellevue Post Acute
