Duties

• Deliver personalized patient care by assisting with activities of daily living (ADLs) such as bathing, dressing, grooming, and mobility support.
• Monitor patients’ health status by observing vital signs, managing medication administration, and reporting changes promptly to healthcare professionals.
• Support clients with meal preparation and nutritional needs, ensuring dietary requirements are met with attention to safety and preferences.
• Assist with patient transfers using manual handling techniques and specialized equipment like Hoyer lifts to ensure safe movement.
• Maintain detailed care plans tailored to each individual’s needs, including dementia or Alzheimer’s care, autism support, or neurodevelopmental disorder assistance.
• Provide companionship and emotional support while encouraging social engagement and mental stimulation.
• Uphold residents’ rights by respecting privacy, confidentiality (adhering to HIPAA regulations), and promoting a respectful environment in accordance with home & community care standards.

Qualifications

• Proven experience working with individuals with disabilities, developmental or intellectual disabilities, or seniors in settings such as group homes, assisted living facilities, nursing homes, or home health environments.
• Strong knowledge of patient observation techniques, vital signs monitoring, medication administration, and first aid/CPR certification.
• Familiarity with care procedures including Alzheimer’s/dementia care, memory care strategies, hospice care practices, and patient transfer methods like Hoyer lifts.
• Ability to perform heavy lifting safely and assist with manual handling tasks required for patient transfers or mobility support.
• Excellent communication skills for client interaction and collaboration with healthcare teams; understanding of resident’s rights is essential.
• Experience working with individuals who have neurodevelopmental disorders or autism is highly valued.
• Valid certifications in first aid and CPR; knowledge of HIPAA compliance is required to ensure client confidentiality at all times.
