Hospice Certified Nursing Assistant, CNA

Job Title: Hospice Certified Nursing Assistant (CNA) Location(s): Plymouth, MI and Westland, MI (Plymouth & surrounding areas) Employment Type: Full‑time Employee – 40 hours per week Overview Residential Home Health and Hospice is seeking a compassionate Hospice Certified Nursing Assistant to join our care team. The CNA will collaborate with colleagues to manage and deliver patient care plans in a supportive, team‑based environment. Responsibilities Provide direct patient care and support as outlined in individual care plans. Collaborate with interdisciplinary team members to ensure quality care delivery. Support patients in Plymouth and surrounding areas, including travel to patient locations. Qualifications Certified Nursing Assistant (CNA) certification in the state of employment. High school diploma or equivalent preferred. Hospice experience preferred. Current driver’s license and ability to travel approximately 20% of the day to patient locations. Demonstrated ability to self‑manage time and schedule. Experience with tablets, mobile phones, and electronic medical record (EMR) software.
