Personal Touch Home Care is expanding our Special Needs Program and seeking compassionate Occupational Therapists who are passionate about delivering high-quality care in the home setting.

This is fee-for-service position, paid per visit and this is a 1099, contracted position. In order to be considered for this position, you MUST have a valid and active license in the state of NY .

Pay: $75.00 per visit to $100.00 per visit

Job Summary:  As an Occupational Therapist with Personal Touch Home Care, you will evaluate patients in their homes, develop individualized treatment plans, and provide skilled therapy services that promote independence, mobility, and improved daily functioning. You'll work closely with physicians, patients, families, and interdisciplinary team members to deliver exceptional patient-centered care.

Responsibilities:

1. Responsible for the assessment of referred patients that require rehabilitation services.

2. Providing direct patient care to assess their medical condition, functional capabilities, limitations and restrictions and potential for rehabilitation.

3. Establish and administer a treatment program with specific goals determined according to the patient’s capacity and tolerance under the direction of the Physician.

4. Set realistic and achievable goals for their patients.

5. Document and record the patients’ condition.

6. Educate patients and families in an appropriate therapy method.

7. Evaluations, weekly progress notes, recertification and discharge documentation.

8. Collaborates with all disciplines to provide the best patient centered treatment and care.

9. Perform all other duties assigned by Director of Rehabilitation.

10. Compliance with confidentiality, HIPPA and Federal Residents Rights. 

Qualifications:

• Graduate of an accredited Occupational Therapy program.

• Current, active New York State Occupational Therapist license required.

• Home health experience preferred.

• Experience with pediatric, adult, geriatric, or long-term care populations is a plus.

• Excellent communication, documentation, and organizational skills.

• Preferred: Minimum of two (2) years of Occupational Therapy experience.

Job Types: Fee-for-service, per diem
