This role requires strong clinical judgment and experience managing patient caseloads.Must-Have Qualifications:Master's Degree in Social Work (MSW) – RequiredMinimum 1+ year of acute care hospital experienceTravel experience required (No first-time travelers – HARD STOP)Strong knowledge of care coordination and discharge planningAbility to manage caseload of 20–23 patientsPreferred Qualifications:Experience in hospital case management or social servicesStrong communication and interdisciplinary collaboration skillsFamiliarity with hospital workflows and patient advocacyWork Environment:Acute Care HospitalCase Management / Social Services DepartmentOnsite role in Mission Viejo, CAPatient Caseload: 20–23 patientsAdditional Requirements:Must be comfortable floating to Laguna facilityRTO Restriction: Maximum 7 daysDress Code: Turquoise scrubsLicense: Not required at submission, must be active by start date if applicableMust comply with all clinical and regulatory standards
