Develop individualized plan of care for each client that incorporates data analysis and current scientific findings. Provide interventions focused on achieving realistic client outcomes. Report changes to update the plan of care to reflect progress toward goals and outcomes. Administer medications as needed. Maintain client records showing systematic assessments, planning interventions, and evaluations. Initiate referrals to other health team members as needed. What we’re looking for Graduate of an accredited school of professional nursing. Current license as a Registered Nurse (RN). Current CPR certification. Reliable transportation. Maintains confidentiality in all aspects of the job. Knowledge of safety and infection control practices.
