In this role, you will: Conduct thorough evaluations for patients in both inpatient and outpatient settings and create tailored treatment plans. Implement evidence-based physical therapy interventions focusing on improving mobility, strength, balance, and overall function. Work collaboratively with fellow nurses, physicians, case managers, and other rehab professionals to ensure the best outcomes for your patients. Educate patients and their families regarding therapeutic goals, discharge plans, and home exercise programs. Maintain accurate documentation of evaluations, treatments, and patient progress in compliance with facility standards and regulatory requirements. Actively engage in care team rounds and interdisciplinary meetings to coordinate and enhance patient care. Educational Requirements: A Doctorate in Physical Therapy (DPT) or equivalent degree from an accredited institution. Certifications: Valid state licensure as a Physical Therapist. CPR certification preferred. Additional specialized certifications (such as neurological or geriatric therapy) are a bonus, but not mandatory. Skills: Previous experience in a similar environment is highly preferred. Proficient in treating patients with diverse medical and mobility challenges. Strong interpersonal and teamwork skills.
