Community Health Navigator & Advocate
Carle Health
Carle Health in Illinois seeks a Community Health Worker (CHW) to partner with community agencies, support at-risk individuals, and help navigate health and social services. The CHW will build bridges between underserved communities and resources, conduct home visits, and participate in group sessions and health fairs as needed. Under supervision, the CHW will coordinate health education, disease management, and case management, promoting health equity, guiding clients to needed services, and #J-18808-Ljbffr Carle Health
