Ultimate Therapy is seeking a Physical Therapist (PT) to join the rehabilitation team at Autumn Lake at Calvert Manor. This is an opportunity to provide individualized therapy while helping residents improve mobility, strength, and overall function in a supportive skilled nursing and post-acute care environment.
What You'll Do

• Perform physical therapy evaluations and develop individualized plans of care
• Provide skilled treatment focused on strength, balance, gait, transfers, endurance, and mobility
• Establish functional goals and monitor resident progress throughout treatment
• Modify treatment approaches based on clinical needs and response to therapy
• Educate residents and caregivers on exercises, mobility techniques, and safety strategies
• Complete accurate and timely clinical documentation
• Collaborate with PTA, OT, SLP, nursing, and other members of the interdisciplinary team

Qualifications

• Active Maryland Physical Therapist license required
• Graduate of an accredited Physical Therapy program
• Strong clinical judgment, communication, and teamwork skills
• Skilled nursing or rehabilitation experience preferred, but not required
• New graduates are welcome to apply
