Responsibilities Plan and administer prescribed physical therapy treatment for patients, working onsite in their homes, to attain highest level of physical function. Assess and document patients\' physical strengths to support physician evaluation and guide treatment goals. Develop and implement a rehabilitation program consistent with physician\'s Plan of Treatment and the goals of the patient team. Provide PTAs and Home Health Aides with clear written care instructions and oversee their performance and documentation. Document patient observations, interventions, and evaluations promptly and thoroughly. Report patient progress to the physician, Clinical Manager, care team, patient, and family, and submit all required evaluations, plans, progress notes, and discharge summaries. Qualifications Required Current and unrestricted Physical Therapy license Current CPR certification Experience collaborating with a team of healthcare professionals Preferred One year of experience as a Physical Therapist Compensation and Benefits Pay per visit / unit rate: $59.00 - $83.00 Pay range: $93,000 - $128,000 per year (full-time) Description of Benefits: Medical, dental and vision benefits, 401(k), paid time off and holidays, disability, life insurance, and more. Compliance and Scheduling Scheduled Weekly Hours: 40 TB Statement: This role is part of Humana\'s TB screening program and requires TB screening if selected. Driving Statement: Requires a valid state driver\'s license and personal vehicle liability insurance as per state requirements.
