MULTI-EMPLOYER ROLE

This W-2 hourly Family Nurse Practitioner position includes responsibilities that span across the following three organizations, all under shared leadership:

FOCUS #1 Community Wellness Outpatient Mental Health Center

FOCUS #2 Lighthouse Behavioral Health Center

FOCUS #3 Freedom Healthcare

You will maintain separate W-2 employment relationships with each organization and will receive individual payroll compensation from each entity based on the time worked and services provided for their respective programs. This means you will be an official employee of all three companies, with hours, responsibilities, and compliance obligations tracked separately for each.

This role is structured as a 24-hour per week position, with approximately one-third of the total weekly hours (about 8 hours per week) allocated to each organization. While day-to-day supervision and scheduling will be coordinated centrally to support integrated care, each employer will manage its own onboarding, HR, and compliance processes in accordance with applicable laws and organizational policies.

CLASSIFICATION

This is a part-time, W-2 employee position totaling 24 hours per week, worked over three scheduled days, with hours allocated across three affiliated companies as follows:

• Freedom Health Care - Freestanding General Primary Care Clinic (FHFSGC):
  Part-time, W-2 employee - approximately 2.4 hours per week (10% of total weekly hours)
• Community Wellness Outpatient Mental Health Center (CWOMHC):
  Part-time, W-2 employee - approximately 10.8 hours per week (45% of total weekly hours)
• Lighthouse Behavioral Health Center (LHBHC):
  Part-time, W-2 employee - approximately 10.8 hours per week (45% of total weekly hours)

This role is one of five individuals collectively assigned to meet and exceed the COMAR-mandated leadership coverage requirements, which include:

• 20 hours per week for a Program Director at Community Wellness OMHC
• 20 hours per week for a Medical Director at Community Wellness OMHC
• 20 hours per week for a Clinical Director at Lighthouse Behavioral Health Center

Collectively, the five assigned professionals provide a total of 120 hours of leadership coverage per week, which exceeds the COMAR requirements for each organization as follows:

• Community Wellness OMHC (Backup Program Director Requirement):
  COMAR requires 20 hours/week; the team provides 24 hours/week
  → Exceeds requirement by 4 hours/week
• Community Wellness OMHC (Medical Director Requirement):
  COMAR requires 20 hours/week; the team provides 54 hours/week
  → Exceeds requirement by 34 hours/week
• Lighthouse Behavioral Health Center (Clinical Director Requirement):
  COMAR requires 20 hours/week; the team provides 42 hours/week
  → Exceeds requirement by 22 hours/week

This structure ensures robust clinical leadership and regulatory compliance across all entities, with ample coverage to support oversight, team collaboration, and quality improvement initiatives.

WORK SCHEDULE

The Psychiatric Mental Health Nurse Practitioner (PMHNP) will work every Monday, Friday and one other set weekday each week, from 8:00 AM to 5:00 PM on each assigned day. The schedule will include one full administrative day (Fridays) and two days (Thursday and one other set weekday) focused primarily on direct client care and clinical services.

• Friday of each week is designated as the Administrative Day, with structured responsibilities that include leadership meetings, documentation audits, quality reviews (COMAR, CARF, policies), psychiatric updates (90792), and team collaboration.
• The Friday and one other set weekdays will focus on the provision of psychiatric services, including:
  • Psychiatric focused evaluations and medication management
  • Substance use disorder focused evaluations and medication management
  • Primary care focused evaluations and medication management
  • Care coordination and supervision of multidisciplinary clinical staff
  • Documentation, prescribing, treatment planning, and compliance with CWOMHC / LHBHC / FHFGC service standards

COMPENSATION RANGE
Compensation for this position ranges from $30.00 to $100.00 per hour, and is commensurate with experience, expertise, verified credentials, and available company budget.

ANTICIPATED TRAVEL: 5% of the time. There is no additional compensation for travel.


DUTIES AND RESPONSIBILITIES
1. Facilitate and document coordination with external primary care providers, as clinically appropriate and with proper consent.
2. Prescribe medications following a full face-to-face evaluation and assess for any contraindications prior to initiating a medication regimen.
3. Educate clients and, when appropriate, caregivers on the purpose, effects, and risks of prescribed medications.
4. Monitor and adjust medication regimens as clinically indicated; conduct evaluations at a minimum every 90 days for ongoing medication management.
5. Notify the client's primary care provider of new prescriptions or medication changes every six months or as clinically required.
6. Conduct peer reviews of other prescribers' work and support documentation compliance across the clinical team.
7. Participate in interdisciplinary team meetings with HR, Scheduling, Patient Access, and other departments to support clinical and administrative needs.
8. Conduct batch documentation audits, including review and signature of referrals, treatment plans, biopsychosocial, ASAMs, and intern notes.
9. Collaborate with external agencies or treatment providers in cases involving shared clients or complex care needs.
10. Accept and review incoming lab results in ICANotes; use clinical judgment to adjust care plans accordingly.
11. Ensure all documentation is completed within expected timeframes using ICANotes and is coded appropriately using evaluation and management CPT codes.
12. Contribute to quality assurance, audit readiness, and performance improvement initiatives to ensure compliance with COMAR, CARF, Medicaid, and internal standards.
13. Provide support and education to clinical staff related to medication protocols and service delivery expectations.
14. Submit periodic reports and summaries related to administrative and clinical oversight duties.

15. Administrative Leadership - Friday (8:00 AM - 5:00 PM)

Fridays are fully dedicated to fulfilling administrative and clinical leadership responsibilities across all three affiliated programs:

• Community Wellness Outpatient Mental Health Center (OMHC)
• Lighthouse Behavioral Health Center
• Freedom Healthcare - Community Wellness Primary Care Clinic

The PMHNP will support clinical compliance, supervision, documentation review, quality assurance, and coordination with executive and program leadership in alignment with all applicable COMAR regulations, CARF standards, and internal policies.

During clinical days, the PMHNP will provide psychiatric care, medication-assisted treatment (MAT), and integrated primary care coordination across the following affiliated programs:

• Community Wellness Outpatient Mental Health Center (CWOMHC)
• Lighthouse Behavioral Health Center
• Freedom Healthcare - Community Wellness Primary Care Clinic (a freestanding general clinic)

The schedule for these dedicated clinical days include:

• Conducting psychiatric evaluations, re-evaluations, and follow-up medication management visits for individuals receiving outpatient mental health, substance use disorder, or primary care services
• Delivering Medication Assisted Treatment (MAT) using FDA-approved medications such as buprenorphine, in alignment with COMAR 10.09.80 and federal MAT standards
• Responding to medical emergencies in outpatient or residential care settings and conducting diagnostic assessments as clinically indicated
• Managing and documenting all services in the electronic health record (EHR) using appropriate billing codes, including:
  • 90791, 90792, 99202-99205, 99211-99215, H0004, H0032
• Coordinating care across multidisciplinary teams and external providers to ensure continuity of care and comprehensive support for physical and behavioral health needs
• Serving as the Supervising Provider on Duty during all scheduled shifts for both CWOMHC and Lighthouse Behavioral Health Center, providing clinical oversight for therapists, nurses, prescribers, and support staff
• Referring clients to the Psychiatric Rehabilitation Program (PRP) when clinically appropriate and completing necessary documentation and care transition planning
• Supporting and, when assigned, delivering basic primary care services at the Freedom Healthcare freestanding general clinic, including chronic disease management, health screenings, and coordination with behavioral health providers
• Ensuring that all services adhere to the standards outlined in COMAR 10.21.20, 10.09.80, 10.47.01, and any applicable state and federal healthcare regulations

PHYSICAL DEMANDS: Regularly walk, stand or stoop; occasionally lift, carry, push, pull or otherwise move objects weighing up to 25 pounds; and regularly drive a motor vehicle.


WORKING CONDITIONS: All work is performed in an office where staff is exposed to communicable diseases of patients.

SUMMARY OF POSITION RESPONSIBILITIES


The Psychiatric Mental Health Nurse Practitioner (PMHNP) serves as a critical clinical leader at Community Wellness Outpatient Mental Health Center (CWOMHC), providing direct psychiatric care, medication management, clinical oversight, and regulatory leadership in compliance with COMAR 10.21.20.

In accordance with COMAR 10.21.20.10, this position may fulfill the responsibilities of both the Medical Director and Program Director when applicable. As Medical Director, the PMHNP assumes responsibility for the therapeutic direction and medical quality of the program, including diagnostic and treatment standards, prescribing practices, physician coverage, and clinical supervision. As Program Director, the PMHNP ensures administrative continuity, regulatory compliance, and multidisciplinary team oversight, including during staff absences or vacancies.

Additionally, the PMHNP provides direct psychiatric care and is authorized to bill for a wide range of clinical and rehabilitative services, including but not limited to CPT and HCPCS codes: 90791, 90792, 90832, 90833, 90834, 90836, 90846, 90847, 90853, 99202-99205, 99211-99215, 99417 H0032, H0046, H2027, H1011, S9480, GAMDC.

This role supports integrated treatment planning for individuals with mental health and co-occurring substance use disorders, ensuring care is person-centered, coordinated, and in full alignment with state regulations.

SCHEDULED DUTIES AND RESPONSIBILITIES
1. Direct Clinical Care & Medication Management

• Conduct face-to-face psychiatric evaluations and re-evaluations for individuals receiving services at the OMHC
• Prescribe, monitor, and adjust psychotropic medications based on clinical assessment and client needs
• Provide psychoeducation on medication effects, side effects, and treatment adherence
• Evaluate clients face-to-face at least every 90 days when under ongoing medication management
• Ensure documentation meets COMAR requirements, including rationale for prescriptions, lab monitoring, and coordination with primary care providers
• Notify primary care providers every 6 months regarding medication changes, with proper consent
• Use approved codes for billing all reimbursable services, in accordance with Medicaid and COMAR guidelines

2. Oversight of Co-Occurring Disorder Services (COMAR 10.21.20.08D)

• Review assessments to determine if clients have secondary substance use disorders
• If the program provides substance abuse services, ensure appropriate documentation reflects integrated care
• If not, document coordination with certified substance use providers and maintain collaboration with proper consent

3. Medical Director Duties (Per COMAR 10.21.20.10(2))

• Establish and maintain clinical and prescribing standards for the program
• Review and sign the Medical Director attestation or affidavit required by the Behavioral Health Administration (BHA) or other governing bodies at the time of OMHC program re-licensure or license renewal.
• Serve as the supervising provider for all OMHC services rendered while on duty, providing clinical oversight and availability to other prescribers, therapists, and administrative staff for case consultation, medication or treatment approvals, and emergent decision-making.
• Refer clients to the Psychiatric Rehabilitation Program (PRP) on behalf of the OMHC when clinically indicated, ensuring a smooth and compliant linkage between outpatient mental health services and community-based rehabilitative support.
• Oversee medical aspects of quality management, including peer review and incident response
• Ensure adequate physician or prescriber coverage and after-hours care protocols
• Supervise treatment staff and maintain clinical compliance across disciplines
• Provide consultation to all clinical personnel on complex cases or clinical decisions

4. Program Director Duties (When Designated)

• In the absence or vacancy of the OMHC Program Director, the employee will automatically serve as Acting Program Director. The employee will be responsible for the oversight of clinical operations, supervision of staff, regulatory compliance, program performance, and coordination with external partners and oversight bodies.
• Fulfill all administrative oversight responsibilities under COMAR 10.21.17 and 10.21.20
• Ensure continuity of care and supervision in the absence of other clinical or administrative staff

SUMMARY OF POSITION RESPONSIBILITIES

The Psychiatric Mental Health Nurse Practitioner (PMHNP) at Lighthouse Behavioral Health Center serves a dual role as both a clinical provider and program leader within a licensed behavioral health and residential substance use disorder treatment setting. The PMHNP provides Medication Assisted Treatment (MAT) and psychiatric care, ensures regulatory and quality compliance, and may function in clinical leadership and administrative capacities in accordance with COMAR regulations.

Under COMAR 10.09.80.05, the PMHNP must hold DEA Schedule III authority and is authorized to bill for services using H0004, 99202 HG, 99203 HG, 99204 HG, 99205 HG, 99211 HG, 99212 HG, 99213 HG, and 99215 HG. The PMHNP delivers pharmacologic treatment using FDA-approved medications such as buprenorphine, supports flexible tapering and withdrawal management, and ensures that all services are clinically appropriate and client-centered.

The PMHNP also fulfills requirements for residential Level 3.1 (low intensity), Level 3.3 (medium intensity) and Level 3.5 (high intensity) providers by ensuring sufficient clinical coverage to conduct initial diagnostic work-ups, manage medical emergencies, and refer for identified medical or surgical needs. The PMHNP must be available for at least:

• 3 hours per week for Level 3.3 services
• 1 hour per week for Level 3.1 and Level 3.5 services

In addition, the PMHNP serves as the Clinical Director as defined under COMAR 10.21.07.02(14), responsible for the therapeutic and rehabilitative direction of the program. This includes direct clinical supervision of counselors, peer recovery staff, and oversight of all care coordination with external providers.

The PMHNP may also serve as the Program Director, and must be on-site at least 20 hours per week while fulfilling that role. The Program Director is responsible for the continuity of clinical operations, regulatory compliance, and staff supervision.

In the absence or vacancy of the Lighthouse Administrator, the PMHNP will automatically assume the responsibilities of Acting Administrator, ensuring uninterrupted program operations, staff leadership, adherence to internal policies and procedures, and regulatory oversight per COMAR 10.47.01.06.

SCHEDULED DUTIES AND RESPONSIBILITIES

1. Medication-Assisted Treatment & Psychiatric Services

• Conduct initial psychiatric and diagnostic evaluations, including history-taking, risk assessment, and treatment planning
• Prescribe and manage FDA-approved medications (e.g., buprenorphine), ensuring dosage is personalized and clinically justified
• Monitor and manage withdrawal symptoms, support stabilization, and prevent relapse through individualized MAT plans
• Educate clients about medication effects, safe use, storage, side effects, and their role in recovery
• Provide flexible tapering and withdrawal management when requested, while maintaining medical safety
• Order and administer non-narcotic medications when indicated
• Ensure thorough, accurate documentation in the EHR, and use correct billing codes as authorized under COMAR and Medicaid

2. Residential Program Compliance

• Ensure sufficient clinical presence for Level 3.3 and Level 3.5 residential services by:
  • Being available at least 3 hours/week for Level 3.3
  • Being available at least 1 hour/week for Level 3.5
• Provide initial diagnostic evaluations, handle medical emergencies, and refer clients for medical/surgical needs as appropriate in residential settings

3. Clinical Leadership (Clinical Director Role per COMAR 10.21.07.02(14))

• Direct and supervise therapeutic and rehabilitative services
• Provide clinical oversight to counselors, peer support staff, and case managers
• Ensure care coordination across providers and promote continuity of treatment
• Maintain clinical documentation and program service quality to meet or exceed licensing standards

4. Program Oversight (Program Director Role)

• Serve as Program Director if designated, and be on-site at least 20 hours/week
• Ensure compliance with COMAR 10.21.17 and 10.47.01, including clinical and operational readiness
• Ensure continuity of care when clinical staff are unavailable
• Supervise multidisciplinary staff and provide mentorship or oversight as needed
• Maintain knowledge of all regulatory requirements related to OMHC and SUD treatment

5. Contingency Administrative Leadership (Acting Administrator)

• Automatically assume the Administrator role in the event of a vacancy

The PCNP will independently assess, diagnose, and treat patients in accordance with established clinical protocols, while also collaborating with medical assistants, behavioral health providers, case managers, and external specialists to ensure whole-person care. SCHEDULED DUTIES AND RESPONSIBILITIES

• Provide comprehensive primary care services, including routine wellness exams, acute visits, chronic disease management, and follow-up care
• Perform patient assessments, obtain medical histories, conduct physical exams, and order diagnostic tests as needed
• Diagnose and treat common acute and chronic conditions such as hypertension, diabetes, asthma, infections, and minor injuries
• Prescribe medications and develop individualized care plans in line with current clinical guidelines
• Deliver preventive health screenings and educate patients on lifestyle changes, immunizations, and disease prevention
• Coordinate care with internal behavioral health, substance use, and case management teams as appropriate
• Make referrals to specialty providers, hospitals, or community-based resources when medically indicated
• Document all encounters accurately in the electronic health record (EHR) and ensure compliance with regulatory and billing requirements
• Participate in team huddles, case reviews, and clinical quality improvement initiatives
• Serve as a clinical resource to support staff and promote evidence-based practice within the care team
