This Home Health RN role is based in St. Louis, Missouri, a dynamic and welcoming city on the Mississippi River known for its iconic Gateway Arch, friendly communities, and affordable cost of living. Forest Park, one of the largest urban parks in the country, offers free world-class attractions including a zoo, art museum, history museum, and miles of walking and biking paths, providing plenty of options for work-life balance and outdoor recreation.As a Home Health RN, you will provide one-on-one nursing care to adult and geriatric patients in their homes throughout the greater St. Louis area, including both city neighborhoods and nearby suburbs. You will be a key member of an interdisciplinary team, focusing on helping patients safely remain in their homes, reduce hospitalizations, and maintain or improve their quality of life.Typical responsibilities include completing start-of-care assessments, routine and follow-up visits, and discharge visits

performing comprehensive assessments

developing and updating individualized plans of care

administering medications and treatments

managing wound care

monitoring chronic conditions

and providing extensive patient and family education. You will collaborate closely with physicians, therapists, social workers, and home health aides to coordinate care and ensure that each patient's needs are met.Your day will typically involve traveling between several patient homes, performing visits that include assessment, intervention, and documentation. Patient caseloads and daily visit expectations are designed to allow thorough, quality care and timely documentation. You can expect to see a variety of conditions, including post-acute surgical patients, those with cardiopulmonary diseases, diabetes, neurological conditions, and patients requiring palliative support. You will use an electronic medical record system for all documentation and communication.Shifts are generally daytime hours on weekdays, with the possibility of participation in a rotating call schedule, weekend coverage, or evening visits depending on agency needs. The work environment is field-based and autonomous, but supported by a collaborative office team that includes clinical leadership and support staff who assist with scheduling, coordination, and resource management.This role is an excellent fit for nurses who value independence, patient-centered care, and the opportunity to make a meaningful difference in patients' lives by caring for them where they are most comfortable.
