MDS Coordinator

Genesis HCC is seeking an experienced MDS Coordinator, RN to lead the Resident Assessment Instrument (RAI) process in our skilled nursing and long-term care center. The MDS Coordinator oversees timely and accurate completion of MDS assessments, care area assessments, and care plans in accordance with Medicare/Medicaid and regulatory guidelines. This role collaborates closely with nursing, rehab, and interdisciplinary teams to ensure accurate clinical documentation, optimize reimbursement, and support high-quality, patient-centered care. The RN provides staff education on MDS, audits documentation, and supports survey readiness and quality improvement initiatives.
