Occupational Therapist

Job Summary:

The Occupational Therapist (OT) provides skilled therapeutic services to residents in a post-acute and long-term care setting. The OT evaluates residents, develops individualized treatment plans, and implements interventions designed to improve function, activities of daily living (ADLs), and safety. Collaboration with nursing and rehabilitation staff is essential to ensure optimal patient outcomes. The role demands strong clinical and interpersonal skills, with a focus on enhancing residents' independence and quality of life. Responsibilities:

• Evaluate residents to assess functional abilities and needs.
• Develop and implement individualized treatment plans tailored to resident goals.
• Provide therapeutic interventions to improve ADLs, mobility, and safety.
• Collaborate closely with nursing, rehabilitation, and interdisciplinary teams to coordinate care.
• Maintain accurate and timely documentation in compliance with state and federal regulations.
• Participate in team meetings and contribute to quality improvement initiatives.

Required Skills & Certifications:

• Current California Occupational Therapist (OT) license.
• Strong interpersonal and clinical skills.
• Commitment to delivering quality care and achieving positive patient outcomes.
