What You'll Do

• Screen and evaluate patients with cognitive or ADL physical functioning disorders
• Develop individualized care plans following regulatory and clinical standards
• Provide rehabilitative, skilled, and medically necessary treatments, including ADL retraining, therapeutic activities, exercises, modalities, and use of supportive devices
• Monitor patient progress and adjust treatment plans accordingly
• Document services accurately in electronic records and maintain compliance with regulations
• Communicate regularly with the healthcare team regarding patient progress and barriers
• Participate in care conferences, trainings, and meetings as required
• Supervise and support rehab aides, therapy assistants, or students as applicable

What You Need

• Valid Occupational Therapist license and NBCOT certification
• At least 1 year of relevant clinical experience
• Strong assessment, communication, and patient care skills
• Ability to follow regulatory standards and documentation requirements
