If you're excited to shape the future of home-based care, here's what the role involves.Saline, MichiganPosition OverviewThe Physical Therapist Per Visit provides physical therapy services; communicates the patient's progress with other disciplines and directs, supervises and instructs non-professional home health aide staff in the provision of personal care to the patient.Essential Duties and ResponsibilitiesUnder the physician's order, admits patients eligible for home care services within 24-48 hoursAssess and evaluates patient needs/problems, identifies mutually agreed upon goals with patientsReports patient status and need for other disciplines to agency Clinical Supervisor and referring physicianReports to assigned follow-up Clinician as indicatedDevelops patient care plan that specifically addresses identified patient problems and goalsUpdates care plans on an ongoing basis; revises and resolves patient problems and goals as changes occur and/or at recertificationCompletes admission paperwork to include checklist and submits to Clinical Supervisor per agency policyProvides physical therapist services including assessment, evaluation, procedures, teaching and training activities as outlined in the patient Plan of CareProvides Physical therapist visits according to visit schedule and notifies agency of need to alter schedule in any wayReports significant findings to patient's physician and Clinical Supervisor as they occurRe-assess patient for effectiveness related to goals, continuing or discontinuing service, changes to goals or care plan update, objective evidence or statement of expectation of continued progress toward goals every 30 daysSubmits completed physical therapist visit notes; communication notes and home health aide supervisory notes per agency policy on designated days as requested by Clinical SupervisorSubmits change orders per agency policyMay supervise the home health aide in lieu of a registered nurse if no nursing services are currently being providedSupervises the Physical Therapy Assistant(s) as applicable and adheres to all rules, regulations, code of ethics, guidelines, and codes in federal, state, and local laws which are concerned with the practice of physical therapyPerforms all OASIS time point assessments per Medicare Criteria and per agency policy and procedureMaintains open lines of communications to all members of the continuum of care teamActs as a preceptor in the orientation of new physical therapy staff as requestedAttends staff meetings, team conferences and educational in-services per agency requirementsParticipates in Process Improvement (PI) program by assisting with collection of data and serves on PI team upon requestParticipates in discharge planning process per Medicare Criteria and agency policy and procedureFollows agency policies and proceduresPerforms these and all other duties as assigned by the AdministratorAble to lift 40 pounds from floor to shoulderRepetitive walking, standing, sitting, bending, and use of hands.Maintains a productivity level of 90%Responsible to ensure the use of the 4Ms (What Matters to the patient, Medications, Mentation, and Mobility) and provides Age-Friendly CareOther duties as assignedRequired Knowledge, Skills and ExperienceA current unencumbered State professional licenseActive CPR CertificationMust maintain a valid driver's license and good driving recordAn automobile to be used for workOne year experience as a home care professional physical therapist and competent in performing comprehensive assessment and OASIS preferredThe ability to make sound professional clinical judgmentThe ability to assess and document patient needs and formulate individualized patient care plans to meet those needsProficiency in clinical skillsProficiency in personal computer use, including email, clinical, word processing, and spreadsheet softwareAbility to drive in a car 2-4 hours per dayPerks and Benefits (What we offer):Competitive salary and multiple health benefit optionsCareer growth and developmentMentorship and continued learning opportunitiesEngaging and mission driven workplace401(k) matchingMileage reimbursement (if eligible)Every person on our team helps shape the future of Alpine Care Home Health.
