Our team of caregivers, nurses and therapists work with the guidance and collaboration of the patient's medical team to provide the services needed for their recovery.About CareageCareage is a leading provider of senior-focused construction, management, and health care services throughout the United States. They are committed to providing exceptional services to a wide variety of clients, including hospitals, medical clinics, skilled nursing and post-acute rehabilitation facilities, Assisted Living communities, Memory Care centers, and retirement communities.A Registered Physical Therapist (PT) administers physical therapy to patients on an intermittent basis in their place of residence. This is performed in accordance with physician orders and plan of care under the direction and supervision of the Director of Clinical Services.Summary of Responsibilities:Travel within Thurston County to provide care on an intermittent basis in patients' homes.Provide physician orders and plan of care under the direction and supervision of the Director of Clinical Services.Work in conjunction with all other home care personnel in patient care planning.Direct and supervise Physical Therapy Assistant and Home Health Aide, as assigned.Take initial patient history and make initial evaluation.Consult with physicians regarding change in treatment.Instruct patients and family in home programs and activities of daily living.Participates in Quality Assessment and Performance Improvement activities as assigned.Complete and submit OASIS assessments, reassessments, transfers, resumptions of care, discharges and significant change in condition.Appropriately utilizes ICD-10 codes.RequirementsGraduate from a physical therapist curriculum approved by the American Physical Therapy Association.Currently licensed as a physical therapist in the state of Washington.Two (2) years' experience as a physical therapist, preferred.Home Health experience, preferred.Detail oriented, self motivated and capable of efficient time management.Critical thinker capable of working collaboratively with a team.Valid WA State driver's license.
